Evidence Health
Are signals flowing — and is anyone reading them?
What percentage of your teams emit the signals they've committed to in their evidence contracts?
Does every emitted signal have a named reader-of-record — someone responsible for the constitutional read?
When evidence contradicts something leadership believed, what happens?
How quickly does a structural signal — something the spine should respond to — produce a change?
Can you identify ghost signals — decisions that should have generated evidence but didn't?
Execution Health
Is work happening within constraints, or around them?
If a new cell plugs into your organisation today, what constraints does it inherit automatically — without anyone sending a Slack message?
How do your teams coordinate — primarily through artefacts or through meetings?
Can you find a decision from last week that nobody remembers making? (A ghost decision.)
What is the agent-to-human action ratio for routine work — and is it what you'd expect?
If your most senior spine author left tomorrow, which decisions would become unmade?

Is ISEE making the organisation faster, safer, and more traceable?
What percentage of engineering time goes to coordination versus execution?
How long does it take a new cell to ship its first change?
When an incident occurs, can you trace it to: a spine gap, an evidence gap, or an execution failure?
